HMS Monmouth Safety Case

BMT was tasked by the Frigates Integrated Project Team (FIPT) to produce a Safety Case Report (SCR) for the Type 23 Frigate HMS MONMOUTH in accordance with Joint Services Publication (JSP) 430 - Ministry of Defence (MoD) Ship Safety Management Policy.

The Requirement

BMT was tasked by the Frigates Integrated Project Team (FIPT) to produce a Safety Case Report (SCR) for the Type 23 Frigate HMS MONMOUTH in accordance with Joint Services Publication (JSP) 430 - Ministry of Defence (MoD) Ship Safety Management Policy. The SCR was to be applicable to the design and material state of the vessel at the end of Refit Period 1.

The SCR aimed to provide the Duty Holder (who is responsible for the capability of the vessel), and the Stakeholders (who use the capability), with the visibility and assurance that safety is being managed effectively. The SCR was required to provide the source of key safety information to enable:

  • Safe operation of the ship within the safe design limitations; 
  • Operation of the ship within legal requirements of the MoD and other statutory legislation;
  • Operation within a defined Safety Management System (SMS) 

Our Approach

BMT applied a structured approach to the production of the SCR, ensuring that activities were conducted in order to support and provide safety assurance on specific aspects of the Safety Baseline design. This was achieved by undertaking a Top Level Safety Assessment with the objective of identifying credible hazards, and where appropriate, reducing the residual risk from these hazards to As Low As Reasonably Practicable (ALARP). Tasks undertaken included:

  • Top Level Hazard Safety Assessments of whole ship systems and equipment; 
  • Major Hazard Areas Identified by previous study; 
  • Environmental Assessments

Outcomes and Benefits

HMS MONMOUTH's SCR was produced on completion of RP1 to meet FIPT’s responsibilities (as Duty Holder) for Safety Management under JSP430. The SCR provided the Duty Holder and Stakeholders with assurance that evidence exists to demonstrate that the safety of the design and the material state is being effectively managed and that HMS MONMOUTH is safe to continue operational duties.

The SCR was endorsed by the FIPT TL following recommendation by the MoD appointed Independent Safety Auditor (ISA) that the SCR was a true summary of the Safety Case, and that an effective system was in place to manage FIPT’s ship safety management responsibilities.
